Work Study

Welcome and thank you for your interest in participating in the Heritage University Work Study Program.The program will allow students to pursue placement of their choice and serve as a tool for students to attain skills in the area of job readiness.

How DOES THE Work Study Placement Work?

  • Students must be awarded Work study. A Financial Aid Representative will award all work-study eligible students and determine the award amount.
  • Visit the job board after you have selected the desired job you wish to pursue. You will need to:
    • Submit your online work-study application here. If an agreement between student and supervisor is made (willingness to hire), the supervisor will inform the Work Study Coordinator.
    • If you are selected for a work-study position you will be contacted by Financial Aid to set up an appointment within 1-3 days to generate a contract. You will be required to attend a new hire orientation by Human Resources to provide necessary payroll documents (please visit http://www.uscis.gov/files/form/i-9.pdf for a complete listing of required documents, which must be presented at the meeting; one item from column A or one item from column B and one item from column C.)

Important FACTS about Work Study Placement:

  • Workstudy awards range from $2,000 - $9,000 for an academic year.
  • Students are allowed to work up to 19 hours per week but under no circumstance may a student work more than 19 hours per week.
  • Individualized placements will determine the hour allotment per week as well as the hourly wage.
  • Placement is determined by supervisor
  • Placement is limited, not all Workstudy Awarded students are guaranteed a placement.
  • Students who are hired through the workstudy program must maintain academic satisfactory progress
  • Students placed through the program are recommended to attend resume writing workshops, career fairs and participate in mock interviews

2. Eligible for State and/or Federal funding

3. GPA 2.0 or higher

4. Attending 1/2 time (6 credits) or greater

JOB INFORMATION
Title of Position: Service Care Coordinator
Supervisor's Title: Program Coordinator II, Office Manager, Program Manager, Program Director, Public Health
Nurse Supervisor-Community Health Services

PURPOSE OF POSITION
This off-campus work-study opportunity allows students to gain hands-on experience while providing direct service, care coordination, and education to patients of Yakima Valley Farm Workers Clinic (YVFWC) across various programs. The position also involves providing information and support to internal and external partners. Students can work up to 19 hours per week, based on eligiblity. 


Essential Functions/Responsibilities/Duties
Executes incoming calls, assesses client/patient needs, and identifies appropriate services that are comprehensive and
individualized to best serve each client/patient.
Collects appropriate documentation to support and determine program services eligibility.
Assists client/patient with scheduling dates for appointments or classes.
Processes patient referrals for a variety of services including Medical, Dental, and programs including Behavioral Health.
Monitors and documents active referrals and client participation.
Assists patients with completion of applications and forms required by the program(s) they will participate in. Ensures patient
information is complete and entered in appropriate data system.
Responds to basic questions from patients.
Provides follow-up contact with patient regarding service and referrals recommended. Encourages client participation and
retention in various programs and classes.
Performs work to align with productivity goals and standards including # patients, # encounters, # appointments scheduled and
# classes facilitated.
Provides program administrative support such as data entry, client file management, report development and contract
compliance.
Coordinates and promotes YVFWC programs. Recruits clients/patients for dental services, medical services, social services or
education classes. Reserves conference rooms, obtains supplies, organizes snacks for classes.
Presents program information in various internal and external community meetings and functions as requested.
Performs other duties as assigned.
Tomando Master Trainer duties (if assigned):
Required to train and support CDSMP Lay Leaders, ensure the fidelity of the curriculum and keep up to date with program
changes, must maintain Tomando Master Trainer Certificate.


QUALIFICATIONS

Required Education
• High School Diploma or General Education Diploma (GED).
Required Experience
• One year of administrative support and customer service experience.
• For CHS only: One year’s health care or social services experience.
Preferred Experience
• Two years’ administrative support experience in a social services or healthcare setting.
Professional Licenses/Certificates/Registration
• Applicable license or certification.
• Some positions may require program specific certification.
• Tomando Master Trainer certification (if assigned Master Trainer duties).
Knowledge/Skills/Abilities Required or Preferred
• Bilingual (English/Spanish) preferred. Must demonstrate the ability to communicate at level 9 on the language proficiency
scale to receive bilingual differential pay.
• Ability to plan, coordinate and organize work projects.
• Ability to provide professional written and verbal presentations.
• Ability to prioritize work and handle a variety of tasks simultaneously, with frequent interruptions.
• Ability to effectively interact with the public, patients, and clinic staff in a professional manner.
• Ability to understand and apply program requirements and applicable local, state and federal guidance.
• Knowledge of the social and health agencies in the area and services they provide.
• Effective verbal, written and listening communication skills.
• Basic proficiency with a variety of computer programs including Microsoft Office, Word and Excel.
Working Conditions and Physical Requirements
• Physical requirements for this position include hearing, seeing, speaking, talking, feeling, stooping, kneeling, crouching,
reaching, fingering, grasping, and repetitive motions.
• This position requires sitting for long periods of time in front of a computer. Walking and/or standing is only required
occasionally.
• This position requires exerting up to 20 pounds of force and/or 10 pounds frequently in order to lift, carry, pull or move
objects.
• Valid Driver’s License and proof of automobile liability insurance coverage.
